CANCELLATIONS & MISSED APPOINTMENTS

YOU MUST NOTIFY OUR OFFICE OF ANY AND ALL CANCELLED APPOINTMENTS WITHIN 24 HOURS. 

If we do not hear from you, it is considered a "missed appointment," and you are responsible for payment in full. 

PATIENT STATUS

If your last visit has exceeded a two year term frame, you will return as a new patient.

INSURANCE

Due to the nature of insurance regulations, we do not accept insurance programs. We are able to provide you with a super bill that you can submit to your insurance company if you should choose.
